LPN or CMA - Laurel Fork, VA

Physician - Floyd, VA

Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center headquartered in Laurel Fork, Virginia, with locations in Ferrum, Floyd, Laurel Fork and Troutdale Virginia. For almost 40 years, Tri-Area has provided care to underserved individuals in Southwest Virginia. We offer integrated primary medical and behavioral health care in all locations and have pharmacies in Ferrum and Laurel Fork. All of our sites are National Committee for Quality Assurance-certified Patient-Centered Medical Homes and medical professionals at all sites are eligible for National Health Service Corps Loan Repayment.

POSITION: We are seeking a Board-certified / Board-eligible Family Medicine Physician to work 3 days a week and join the Floyd Clinic. There is the possibility of this becoming a full-time position when the addition to the clinic is complete (which is anticipated to be December 2022).

This position involves providing complex direct medical care of patients (in person and through technology), including older adults, individuals with two or more chronic medical conditions, and people experiencing difficult social drivers of health. The work is performed independently with oversight provided by the Medical Director. The Physician must operate in accordance with the terms and conditions of the various contracts and grants providing for the funding of programs and projects.

Pay will be based on training and experience. We provide an excellent benefits package.

REQUIREMENTS: Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association or the Committee on Hospitals of the Bureau of Professional Education of the American Osteopathic Association. New graduates are welcome to apply.

Family Nurse Practitioner - Floyd, VA

POSITION: We are seeking a Family Nurse Practitioner to work 3 to 5 days a week (Clinic operating days are Monday through Friday) in our Floyd clinic.

This position involves providing complex direct medical care of patients, including older adults, individuals with two or more chronic medical conditions, and people experiencing difficulty with social determinants of health. The work is performed independently with oversight provided by the Medical Director. The candidate must possess the ability to provide acute care for a variety of needs and be competent in performing related tasks such as minor suturing. The Nurse Practitioner must operate in accordance with the terms and conditions of the various contracts and grants providing the funding for Tri-Area’s programs and projects.

Tri-Area Community Health clinical staff may qualify for National Health Service Corp loan repayment through https://nhsc.hrsa.gov/

Behavioral Health Director - Laurel Fork or Fries, VA

Job Summary

For over 40 years, Tri-Area has provided care to underserved individuals in Southwest Virginia. We offer integrated primary medical and behavioral health care in all these clinic locations. We also have pharmacies that are open in Ferrum and Laurel Fork and a pharmacy in Fries that will open soon. Our three large sites are National Committee for Quality Assurance-certified Patient-Centered Medical Homes and behavioral health professionals at all sites are eligible for National Health Service Corps Loan Repayment (HPSA score of 19).

We are excited to be opening a new site at Carroll County Public Schools.

POSITION: We are seeking a Licensed Clinical Psychologist, Licensed Clinical Social Worker, or Licensed Professional Counselor to serve as Tri-Area’s Behavioral Health Director. The Behavioral Health Director is expected to work 4-5 days a week seeing patients, completing administrative duties, and visiting clinics. The preferred home clinic location is Laurel Fork or Fries. The position requires competency in providing interventions to individuals, families, couples, and groups with behavioral health and/or substance use problems in a primary care setting. Patient care is provided in an integrated model, in collaboration with medical professionals. The Behavioral Health Provider must operate in accordance with the terms and conditions of the various contracts and grants providing the funding for Tri-Area’s programs and projects. As Behavioral Health Director, this person supervises other Behavioral Health Providers and is involved in writing and implementing organizational policies and procedures.

Non-Clinical Opportunities

Behavioral Health Educator & Outreach Worker - Carroll County Schools

This position is primarily responsible for working with school district employees in Carroll County Public Schools, community members, and community organizations regarding the behavioral health of youth. This person will provide assistance and training to school employees, health education to students and family members, and outreach to the community. This is a full-time position (40 hours per week), which may include time during evenings and weekends. Although there may be some work at a Tri-Area Community Health site, most of the duties for this position will be in the schools and community and daily travel throughout the service area is anticipated (mileage will be reimbursed).

Job Requirements

  • Associate degree in human services is required for this position with preference given to those with a Bachelor’s degree in human services.
  • Two years of work experience is required; health or human services experience is preferred but not required.
  • Knowledge of behavioral health, positive attitude toward persons with behavioral health concerns, and willingness to work with youth who have behavioral health issues is required.
  • Excellent interpersonal skills, including the ability to interact professionally and gain trust with culturally diverse individuals, are required.
  • Ability to handle confidential information with discretion and professionalism is required.
  • Computer skills are required.
  • Access to reliable transportation is required.
  • Successful completion of a satisfactory background check is required.
  • Prior experience in a health educator and/or in an outreach worker or similar role is strongly preferred.
  • Bilingual in English and Spanish is strongly preferred.
  • Familiarity with the service area is strongly preferred.
