Nurse Practitioner - Laurel Fork, VA

"Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center headquartered in Laurel Fork, Virginia, with full-service locations currently open in Ferrum, Floyd, Grayson Highlands (Troutdale), Laurel Fork, and Fries, VA. For more than 40 years, Tri-Area has provided care to underserved individuals in Southwest Virginia. We offer integrated primary medical and behavioral health care in all these clinic locations. We also have pharmacies that are open in Ferrum, Laurel Fork, and Fries. Our three large sites are National Committee for Quality Assurance-certified Patient-Centered Medical Homes and medical professionals at all sites are eligible for National Health Service Corps Loan Repayment (HPSA score of 19).

POSITION: We are seeking a Family Nurse Practitioner to work 3 to 5 days a week (Clinic operating days are Monday through Friday) in our Laurel Fork clinic.

This position involves providing complex direct medical care of patients, including older adults, individuals with two or more chronic medical conditions, and people experiencing difficulty with social determinants of health. The work is performed independently with oversight provided by the Medical Director. The candidate must possess the ability to provide acute care for a variety of needs and be competent in performing related tasks such as minor suturing. The Nurse Practitioner must operate in accordance with the terms and conditions of the various contracts and grants providing the funding for Tri-Area’s programs and projects.

Tri-Area Community Health clinical staff may qualify for National Health Service Corp loan repayment through https://nhsc.hrsa.gov/

REQUIREMENTS: Nurse Practitioners must be licensed to practice as a Family Nurse Practitioner in the Commonwealth of Virginia (autonomous strongly preferred).

Psychiatric Mental Health Nurse Practitioner or Physician Assistant - Floyd, VA

Tri-Area Community Health is a Federally Qualified Health Center with locations in Carroll, Floyd, Franklin, Grayson, and Patrick Counties and Galax City. We are currently seeking a full-time Psychiatric Mental Health Nurse Practitioner or Physician Assistant with CAQ-Psychiatry credentials for our Floyd Clinic. This person will be the organization’s primary medical provider in our growing substance use disorder treatment program, including an expectation to be the prescriber for patients participating in Medication-Assisted Treatment. This person also will assist in leading organizational advocacy efforts for patients seeking supportive substance use treatment. The position requires the ability to work collaboratively with medical and behavioral health providers and other healthcare professionals within an integrated primary care setting. In addition to spending most of the time seeing patients in person in Floyd, the position will include providing tele-psychiatry services to patients at other Tri-Area locations. Case Manager/Care Coordinator - Laurel Fork, VA

Tri-Area Community Health Center in Laurel Fork is seeking a full-time Case Manager/Care Coordinator. Experience and a minimum of an associate's degree required, Human Services preferred. The position is responsible for coordinating patient access to care needs. Provides additional support to individual patients. Referrals for specific programs as well as general consults, indigent care, and maintaining a broad referral network through outreach are all components of the position. Collaborative communication and positive working relationships with clinic staff, behavioral health and medical providers, and community partners are necessary skills.

This position is primarily responsible for working with school district employees and students in Grayson County Public Schools, community members, and community organizations regarding the behavioral health of youth. This person will provide assistance and training to school employees, health education to students and family members, and outreach to the community. This is a full-time year-round position (40 hours per week), which may include time during evenings and weekends. Although there may be some work at a Tri-Area Community Health site, most of the duties for this position will be in the schools and community and daily travel throughout the service area is anticipated (mileage will be reimbursed).

  • Two years of work experience in education, health care, or human services is required.

Part-Time Front Office Assistant - Laurel Fork, VA

Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center with locations in Ferrum, Floyd, Laurel Fork, Troutdale, Fries, Galax Schools, Carroll County Schools, VA and Cana, VA. We are currently seeking a part-time front office assistant to work primarily in our Laurel Fork clinic 3 days a week (Monday through Friday) and days may vary. This position requires a high school diploma and one year of receptionist experience is preferred. The applicant should possess basic knowledge of medical terminology, proficient typing skills, and basic computer literacy, interpersonal and communication skills. Some lifting, pushing, pulling, bending, and stooping will be involved. The ideal candidate will have the ability to provide quality, error-free work in a fast-paced environment, have the ability to work independently with minimal on-site supervision, and have excellent customer service skills.

This person will provide assistance and training to school employees, health education to students and family members, and outreach to the community. This is a full-time, year-round position (40 hours per week), which may include occasional evenings and weekends. Although some work may occur in Tri-Area Community Health clinics, most of the duties for this position will be performed in the schools and community. The coverage area includes seven elementary schools throughout Carroll County and daily travel is anticipated (mileage will be reimbursed).
