Tri-Area Community Health telehealth site is conveniently located within the Elk Creek Rescue Squad building.
Funding for the telehealth equipment at this location was provided by the Virginia State Office of Rural Health.

Schedule an Appointment
- Call Tri-Area Community Health at 888-908-4788 to schedule a virtual appointment with Kelly Burris, FNP or another provider. Be sure to inform office personnel that you would like an Elk Creek telehealth appointment.
- Complete online registration forms available on our website at Elk Creek Rescue Squad Online Form. If you do not have internet access, forms may be picked up at the rescue squad building.
- Come to the Elk Creek Squad Building at your appointment time. Paramedic Monica Cornett or Chief Jamie Carpenter will be there to assist with intake and telehealth connection.
- Patients will come to a full-service Tri-Area Clinic for lab work and injections, if needed.
- Appointments available Monday and Wednesday 1:00 - 5:00 pm and Friday 1:00 - 4:30 pm.
- Available to anyone in Grayson, Wythe or surrounding counties.
